Operator's Report of Motor Vehicle Accident

Use this form to report a motor vehicle accident on or after November 1, 2001

Obtain four copies:

• Send the first copy to the police department in the jurisdiction where the accident took place
• Send the second copy to the Registry of Motor Vehicles
• Retain the third copy for your records.
• Send to Insurance Company
